Medicare introduced multiple new billing opportunities for physician practices in 2024. Mzny of  these services focus on  patient centered and value added care for Medicare patients.  For a patient experiencing continuous ongoing care for a single, severe or complex conditions with the same provider/group, CMS developed code G2211..  This add-on code provides additional payments to providers long term monitoring and developing an ongoing relationship with their patients that may improve outcomes. Another opportunity for evaluating patients that may have social issues impacting their healthcare CMS intstituted G0136,Social Determinants of Healthcare and G0019 and G0022 Community health integration (CHI) to helping patients overcome these issues.
